STD Plan ID Here the operator may enter the STD Plan ID from all the current STD Plans. If there are no options listed, you may first need to create one by clicking on Plan List. Yes
Effective Date Here the operator may enter the Effective Date in MM/DD/YYYY. Yes
End Date Here the operator may enter the End Date in MM/DD/YYYY. Optional
Plan List Clicking here opens another window where the operator has the ability enter and edit STD plans.

Save This button closes the STD Plan Entry Screen, saving the entered information, returning you to the Short Term Disability Coverage History Panel. 
Done This button closes the STD Plan Entry Screen, without saving the entered information, returning you to the Short Term Disability Coverage History Panel.
