This report groups and totals its output by provider and network.

Providers will only appear in the report if the Paid Date is within the specified Paid Date Range or if the Earliest Date of Service is within the Incurred Date Range.

The threshold determines the number of providers listed in the report. Because the providers are ordered by total charges, providers with fewer total charges and are outside the threshold, are summarized in a line above the total at the bottom of the report.
