Individuals will only appear in the report if the Paid Date is within the specified Paid Date Range or if the Earliest Date of Service is within the Incurred Date Range.

Only Coverage Factor 1 is included in the Single Loss Fund, Coverage Factors 2 to 6 are all included in the Family Loss Fund Total.

Because the Life Count can change from month to month throughout the year, a month by month Life Count is included.
