This report groups and totals its output by unit, location, and group.

You may specify a Unit ID, Location ID, Group ID, or an individual's SSN to generate the report.

The above Effective Date is a reference to when they started into the COBRA program and is not a reference to the Effective date of the report.

Individuals appear in the report only if the Date Paid is during the Report Run Period specified by entering the effective date and the end date of the report in the Eligibility Reports Screen.

Event Type: 1- Termination of Employment, 2- Reduction of Hours, 3- Death of Employee, 4- Divorce or Legal Separation, 5- Cessation of Dependent Status, 6- Employee Eligible for Medicare.
