Check Date To enter a manual check, you must specify the date in MM/DD/YYYY format here. Yes
Check Amount To enter a manual check, you must specify the amount of that check here. Yes
Add Check to List Clicking here will add a check to the STD check table above. The check wil default to a check number of 0 which will only be changed to a proper check number when it is printed.

New Check Amount Here the operator may enter the new amount of the currently selected check in the table. Yes
Alter Amounts Clicking here will update the currently selected check in the table to the amount specified in the New Check Amount text entry field.

As Of: This is the close claim date in MM/DD/YYYY format. Yes
Close Claim Clicking this will append a close date to the STD claim. If no date is specified in the As Of text entry field, the close date will be computed as the current date.

Save To save, you must first save salary in the STD Coverage History Panel as something other than 0. This button closes the STD Claim Screen, saving the entered information, returning you to the Short Term Disability Coverage History Panel. 
Done This button closes the STD Claim Screen, without saving the entered information, returning you to the Short Term Disability Coverage History Panel.
